Thank you for contacting me about the future of the Transforming Care Programme.
This is an incredibly important programme, which is helping people who need long-term care receive it in their own home, or as close to home as possible. I know that the NHS hopes to reduce the number of patients being cared for in hospital by 35 per cent by March next year, and I fully support this ambition.
I am pleased that my ministerial colleagues have been able to report significant progress being made by the Transforming Care Programme.
Lengthy visits to hospitals can be incredibly distressing and I believe we should all make every effort to help people receive the care they need in the most appropriate environment. This programme is particularly important when it comes to caring for those with learning difficulties or disabilities. One of the core commitments of the NHS mandate is to reduce the gap in quality of life and care between people with learning difficulties and disabilities, and the population as a whole. The Transforming Care Programme is at the heart of this.
I know there has been significant concern about the rate of progress of this programme. My ministerial colleagues have acknowledged this, and I can assure you that the NHS is fully committed to its targets. The number of in-patients receiving long-term care is down to 2,400, and I know that the NHS expects to move a further 900 patients out of hospitals and into community and home care this year.
I am encouraged to hear that the number of people with learning disabilities who are having pre-admission care and reviews has significantly increased this year, which has meant fewer people ending up in hospital in the first place. They are able to get the right care much quicker than before; I hope this progress continues.
More work certainly needs to be done, but I hope you share my enthusiasm for the Government’s investment in social care, and plans to properly integrate health and social care services, which will help all patients receive the most appropriate care for their needs.
